Who can file this Application?

An owner of a contaminated site acquired prior to July 1, 1990, which ceased operating as a petroleum storage or retail business prior to January 1, 1985, and there is petroleum product contamination from a petroleum storage system may complete this application and the attached acknowledgement and send to:

Florida Department of Environmental Protection

The Department of Environmental Protection will review your application and determine eligibility. A facility inspection may be required in order to verify eligibility. The DEP will send you a written decision upon completion of this review, along with further instructions regarding cleanup procedures. Submission of this application does not guarantee that you will receive state assistance.
